Athletic physicals offered by CHS April 30

Area students planning to participate in athletics during the 2016-17 school year can get physicals April 30 at the Clovis High School north gym.

The physicals are $5 and available for students in Clovis Municipal Schools, Clovis Christian Schools, Melrose, Texico, Floyd, House, Portales and Grady.

Students must complete a physical evaluation form, medical history form and an athletic participation form completed and signed by a parent or guardian. Forms are available at CHS campuses and Gattis, Marshall and Yucca middle schools on online at clovis-schools.org.

Physicals will be 8 a.m. for girls entering grades 7-12, 9 a.m. for boys entering grades 7-8 and 10:30 a.m. for boys entering grades 9-12.
